In the case of open rhinoplasty, the surgeon will place the incision across the columella, or the delicate column of tissue that separates the nostrils. The incision will extend into the inside of the nostrils from where the surgeon will retract the skin by separating it from cartilage and bone. The advantage with open surgery is that the surgeon can view the anatomy clearly and correct the anomalies to achieve desirable aesthetic results.
In the case of closed rhinoplasty, the incisions are made inside the nostrils, which means that no scarring is visible from the outside.
